Description:The Minneapolis Medical Research Foundation seeks a full-time Inreach Coordinator to work in the Hennepin County Medical Center’s Positive Care Center (HIV/AIDS Program).

Provides advocacy, support and resource referral to improve clinical adherence and health outcomes for persons living with HIV and AIDS in Minnesota. Identifies gaps in current services and barriers to effective behavior change.
Duties:• Make referrals to appropriate services, including primary HIV medical care, with follow up and ongoing service coordination, as needed.
o Clarify services needed
o Identify services (and agencies) that match client needs.
o Contact referral agencies
o Prepare or assist in preparation of paper work and applications
o Confirm that client received services.
• Conduct project data collection and generate required reports in a timely manner.
• Identify patients who have lapsed from care, or are at risk of doing so, using Hennepin County Medical Center (HCMC) electronic medical records.
o Contact patients identified through this process and assess barriers to care.
• Using the formal enrollment process for Inreach, design and implement behavior change interventions to improve clinical adherence and health outcomes.


Qualifications:Skill, Knowledge & Ability (SKA):
Requires interaction with a diverse population. Skilled in client engagement and advocacy for individuals within bureaucratic and medical environments; must be fluent in both oral and written communication in English; must be proficient in commonly used computer software; skilled in assessment, problem solving and service coordination; must maintain a high level of confidentiality and be able to deal with difficult issues sensitively. Demonstrate a working knowledge of topics including, but not limited to, navigation of health care systems and psychosocial services available to people living with HIV/AIDS in Minnesota. Displays high standards of attendance, punctuality, maintains confidentiality, and manages time effectively.

Education/Experience:
Any equivalent combination of education and experience that provides the required knowledge and skills is qualifying. Typical qualifications would be two (2) years post high school education in the social sciences or a related field and one (1) year relevant experience. Equivalent years of experience working with Ryan White programs may be substituted for post-secondary educational requirements.

Experience working with the HIV community and associated resources is preferred. People living with HIV/AIDS are strongly encouraged to apply, as are patients of HCMC's Positive Care Center.
